A quick look at what a NFSS form is and how to submit one in SimpleLTC.
What is a NFSS Form?
- It initiates all PASRR nursing facility specialized services (NFSS) that are recommended and agreed to at:
- The initial IDT meeting
- or - - A specialized services review meeting
- It is used to request the following nursing facility specialized services:
- Physical therapy (PT)
- Occupational therapy (PT)
- Speech therapy (ST)
- A customized manual wheelchair (CMWC)
- Durable medical equipment (DME), which consists of:
- A gait trainer
- A standing board
- A special needs car seat or travel restraint
- A specialized or treated pressure-reducing support surface mattress
- A positioning wedge
- A prosthetic device
- An orthotic device
When to submit a NFSS form:
- Within 30 days from the date of the initial IDT or specialized services review meeting
When not to submit a NFSS form:
- When the individual is not eligible for Medicaid
- When the individual is not PASRR Positive
- When requesting rehabilitative services or customized power wheelchairs
